What they offer:

• adults (18 years and older) who are having emotional and mental health concerns

• children (under 18 years) who have mental health issues that are affecting their daily life

• older adults (65 years and older) who need assessment, treatment, and rehabilitation for thought and mood disorders or dementia

Wetaskiwin Primary Care Network - Mental Health

What they offer:

• The PCN Mental Health team consists of PCN Social Worker who will provide services, resources, and short-term therapy to children (10+), youth, and adult patients in Wetaskiwin and surrounding area. Walk-in sessions are available daily with limited spots dependent on scheduling. Contact the PCN for further information.

• Intake/Referral Form

• Consent form (if under the age of 18)

How to access services:

• Speak with your Family Physician or Primary Care Nurse or PCN

• Call or come and see us at the PCN

• Multi-disciplinary team member to request a referral.
